Part A: The pathophysiology Describe the pathophysiology of hypoglycaemia, including the risk factors, prevalence, signs and symptoms, treatment and outcomes, with reference to clinical resources and literature to support your explanation. (800 words) Part B: Plan of Care Devise a care plan for this infant in response to the Recommendation component of the ISBAR. (500 words) Part C: Literature review Provide a literature review to support any one component of neonatal care as outlined in your devised care plan or which is outlined in the ISBAR. (1000 words) Scenario Identify Taylor James (B/O Sally James), UR 24658821 Situation Taylor has been brought to the Special Care Baby Unit (SCBU) to have a Blood Sugar Level (BSL) taken at 1 hour of age. The result is 1.2mmol/L. The Birth Suite Midwife and her Father, Stuart, has escorted her. Background Mother Sally is 26 years of age, a primagravida (P1+0) who developed gestational diabetes and required insulin with meals. She found it difficult to manage her blood sugar levels throughout the last 8 weeks of her pregnancy, with commonly high BSLs recorded. Sally was induced for hypertension related to her pregnancy at 38 weeks gestation. The membranes were artificially ruptured 4 hours prior to delivery and she was GBS negative. She received an oxytocin infusion and an insulin infusion with her BSLs within normal ranges during labour. Taylor was born vaginally with Apgar’s of 7 at 1 minute and 8 at 5 minutes. Sally’s partner Stuart was present for the birth. A birth weight of 4.1 kilograms was recorded. Sally had immediate skin to skin contact with Taylor. An attempt at a breast feed was unsuccessful. Assessment Taylor’s observations are within normal limits. Recommendation On paediatric review Taylor is to be provided care in the SCBU immediately to correct her blood sugar level.
